What we do
Europe currently numbers 450 million patients in 27 national health record systems that do not interact or share information. When patients cross borders, their health data stay behind. i2X's mission is to solve this in practice.
We implement the European Electronic Health Record Exchange Format (EEHRxF) in real clinical settings across 12 EU Member States, working with 38 partner organisations to validate, test, and deploy 5 priority data categories defined by the EHDS Regulation. Every implementation is designed to reduce burden on healthcare professionals, not add more.
Interoperability by design
Building systems from the start that speak the same language — not update existing ones. Every i2X implementation is based on HL7 FHIR, IHE profiles, and EU Implementation Guides from day one. The standard shapes the system, not the other way around.
Standards-based
Every implementation grounded in HL7 FHIR, IHE profiles, and EU Implementation Guides. We work directly with IHE Catalyst, HL7 Europe, and the Xt-EHR Joint Action — so what i2X builds in clinical settings feeds back into the European standards that matter.
Real-world implementation
Every Implementation and demonstration site is a hospital or health organisation setting, serving patients. The 35 use cases that demonstrate our work were defined by clinicians, IT teams, and patients — not written in a specification document and handed down. This is where i2X's evidence base lies.
Project scope
i2X runs from April 2025 to March 2029. It has 48 months of in-service implementation across 38 partner organisations in 12 EU Member States. i2X is co-funded by the Digital Europe Programme and coordinated by UpHill Health, Lisbon, Portugal. Of those 12 EU Member States, 10 are actively deploying 5 EEHRxF priority data categories in 14 operational healthcare settings and working on 8 interconnected Work Packages towards a common goal: a European health record format that functions on day-to-day clinical practice, not just on paper.
